Club OS can build a custom weblead form for your gym that can be used on any web page. When a prospect submits the form, a prospect profile will be auto-created in your Club OS system. Their origin will be ‘web’ and marketing source will be “web lead’. 

Stand Alone Form

This type of form is a full webpage that contains all formatting and prospect capture fields. 

Embedded Form

This type of form contains prospect capture fields that can be placed inline on your website. The following fields are recommended. Other fields can be added and mapped to an existing Club OS field.

• Phone (mapped to mobile)

• SMS Opt In (mapped to SMS opt in)

For embedded forms, the following selections need to be submitted (examples included)
